Report a Fender Bender

Before you submit the form below, please read this section in its entirety:
- If involved in a motor vehicle crash with minor damages ($1,500 or less) and no reportable injuries, the crash could be considered “non-reportable” and may not require a police response.
- If your vehicle has struck property that is NOT A VEHICLE, such as a highway sign, bike rack, or a building, please call the Police Department to report the incident, even if the damages are minor. 540-382-4343.
- If your vehicle has struck an unattended parked vehicle, please LEAVE A NOTE with your full legal name, contact information, driver’s license number, insurance provider, and vehicle registration number. You may notify the police to report the incident if you wish. The Police Department will try and assist you with reaching the owner of the parked vehicle. Failure to leave a note could result the criminal charge of Hit and Run.
- If your vehicle has struck another motor vehicle that is attended by the other driver, you can complete an Informational Exchange using the form below if there are no reportable injuries and the damages are minor. All drivers involved need to complete and exchange this form.
- Step 1: Complete all sections of this form using your personal and vehicle information. After completing, click the "Submit" button.
- Step 2: A copy of your responses will be sent to your provided email address. Please forward that email to the other party involved in the motor vehicle crash.
A record of this form will be kept by the Virginia Tech Police Department.
